Northern Arizona Crash Kills 3, Critically Injures Child Airlifted to Phoenix Hospital

Head-on Collision Near Prescott Claims Lives of Father, Child, and Second Child En Route to Hospital

PHOENIX — A devastating crash in northern Arizona Saturday afternoon left three people dead and sent a third child to a Phoenix hospital in critical condition, according to the Yavapai County Sheriff’s Office.

The crash happened around 3 p.m. on Williamson Valley Road near Levie Lane, west of Prescott Regional Airport.

Deputies found Jesse Forman and one child dead at the scene. A second child died while being rushed to the hospital, authorities said. A third child was airlifted to Phoenix and remains in critical condition.

What Caused the Crash?

Investigators say Forman was driving a Toyota 4-Runner with his three children and the children’s mother when he crossed the double yellow lines and crashed head-on into a Jeep SUV.

The Jeep was carrying a woman and her adult son, both of whom were seriously injured and transported to a Flagstaff hospital.

“Our deepest sympathy goes out to the children and families impacted by this horrific crash,” said Sheriff David Rhodes. “I also want to acknowledge the first responders who rushed to help everyone involved.”

The cause of the crash is still under investigation. No further details have been released.
