For patients at two Phoenix-area hospitals that closed their doors last year, just a few more months remain to request and obtain copies of their medical records.
Former patients at Gilbert Hospital or Florence Hospital at Anthem have until June 23 to get the records under a court-approved plan.
For security measures, all of the medical records will be permanently destroyed following the deadline. The hospitals closed last June due to financial issues.
Records can be requested by filling out a medical record request form for either Gilbert Hospital or Florence Hospital at Anthem and emailing it to [email protected].
Patients can also call 480-947-3173 or mail Resolute Commercial at 7201 E. Camelback, Ste. 250, Scottsdale, Arizona.
Appointed last summer as the receiver for the hospitals and overseer of the winding-down process is Resolute Commercial.
Medical records can be picked up at the Resolute Commercial offices once the requests are processed and applicants are notified.
